Terms1. Acceptance of Terms By using the services of TPA Mobile Labs, LLC (“Company,” “We,” “Us,” or “Our”), the client (“You,” “Your,” or “Customer”) agrees to the following Terms & Conditions. These terms outline the responsibilities, limitations, and agreements regarding the services provided by TPA Mobile Labs. 2. Scope of Services TPA Mobile Labs provides mobile laboratory specimen collection and testing services, including but not limited to: ✅ Drug & Alcohol Testing (DOT & Non-DOT) ✅ Phlebotomy (Blood Draws) ✅ DNA & Paternity Testing ✅ STD & Infectious Disease Panels ✅ I-9 Verification & Background Checks ✅ Fingerprinting & Compliance Screening We do not provide medical diagnoses, treatment, or prescriptions. All laboratory tests require authorization from a licensed medical doctor (MD) or another qualified healthcare provider. 3. No Physician-Patient Relationship TPA Mobile Labs is not a medical provider or diagnostic clinic. We do not interpret lab results or provide medical advice. All laboratory tests must be reviewed and authorized by a licensed MD or other qualified healthcare provider before proceeding. Customers are responsible for consulting with a healthcare provider to understand their results and determine any necessary next steps. 4. Client Responsibilities By using TPA Mobile Labs' services, you acknowledge and agree that: ✔ You voluntarily request specimen collection and lab testing services. ✔ You understand that all medical orders must be approved by a licensed physician. ✔ You will provide accurate personal and medical information as required for proper testing. ✔ You assume full responsibility for seeking professional medical guidance regarding test results. ✔ You agree to follow all pre-test and post-test instructions as provided by TPA Mobile Labs or your healthcare provider. 5. Payment & Billing Payment is due at the time of service unless prior arrangements have been made. TPA Mobile Labs accepts insurance, self-pay, and employer-billed services (subject to eligibility). Insurance claims are subject to approval and reimbursement policies of the respective insurer. Clients are responsible for any unpaid balances not covered by insurance. 6. Cancellation & Refund Policy Appointments canceled within 24 hours of the scheduled time may be subject to a cancellation fee. No refunds will be issued for services rendered. If a test is unable to be completed due to insufficient sample collection, a rescheduling fee may apply. 7. Confidentiality & Privacy All client information and test results are protected under HIPAA regulations. TPA Mobile Labs will only disclose information to authorized parties with the client’s consent or as required by law. 8. Liability Disclaimer TPA Mobile Labs is not responsible for: Any medical conditions, complications, or outcomes resulting from lab test results. Delays in test processing due to third-party laboratories or external factors. Any misinterpretation or misuse of test results by the client. Employer or third-party decisions based on drug testing or background check results. 9. Governing Law & Dispute Resolution These Terms & Conditions are governed by the laws of the State of Florida. Any disputes shall be resolved through mediation or arbitration before legal proceedings. 10. Acknowledgment & Agreement By using the services of TPA Mobile Labs, you acknowledge that you have read, understood, and agreed to these Terms & Conditions.
